Your responsibilities as a patient

1. Communication and follow-up

  • If you have questions about the information provided by Vital Leaf or if your condition changes after consulting with one of our physicians, you must contact us or another healthcare provider for further guidance.
  • If you experience unexpected or adverse side effects from any treatment, medication, or healthcare product recommended by our physicians, seek emergency medical attention immediately.
  • Retain all patient records and medication information provided by Vital Leaf to assist other healthcare professionals in case of emergencies.

2. Understanding our services

  • Information published on our website or provided in patient materials, including details about medical cannabis, is for general informational purposes only and should not be considered medical advice.

Our services and fees

Vital Leaf specialises in providing consultations and medical cannabis care for patients with pre-existing conditions where traditional treatments have failed to alleviate symptoms or caused intolerable side effects.

1. Consultations and prescriptions

  • Our physicians, registered with the General Medical Council (GMC), will evaluate your condition and determine if medical cannabis could relieve your symptoms.
  • Prescriptions are issued at the sole discretion of our physicians and are based on your medical history, national guidelines, and individual needs.
  • We cannot guarantee the issuance of a prescription. Additional information from your general practitioner (GP) or other specialists may be required before a final decision is made.

2. Appointments

  • Appointments are subject to availability and are offered at Vital Leaf’s discretion. We cannot guarantee the availability of a specific physician.
  • To schedule an appointment, you must provide a valid photo ID and your National Health Number. Your identity will be verified at the start of each consultation.

3. Medical history and updates

  • You must provide complete and accurate medical history information, including:
    1. A completed intake form via our patient portal.
    2. A signed patient declaration.
    3. A photo ID (required at every visit for verification).
  • Notify Vital Leaf of any changes to your medical status or prescriptions from other providers after registering with us.

4. Treatment evaluation

  • As part of your treatment, you will be required to complete detailed questionnaires about your symptoms and their impact on your daily life. These questionnaires will be reassessed during follow-up consultations, which are essential for continued prescription eligibility.
  • Continued prescriptions are contingent on your cooperation with follow-up consultations and adherence to clinic policies.

5. Prescription dispensing

  • Prescriptions for medical cannabis are fulfilled by licensed pharmacies. Dispensing times may vary.
  • The cost of filling private prescriptions is determined by the pharmacy and is beyond Vital Leaf’s control.
  • You are responsible for deciding whether to fill your prescriptions and for covering associated costs.

6. Fees and payments

  • The consultation fees are listed on our website at the time of booking. Full payment is required before your appointment.
  • VAT is included in our fees where applicable, though consultations themselves are VAT-exempt.

Changes to services or terms

1. Service modifications

Vital Leaf reserves the right to modify services, including appointment availability and physician assignments, without prior notice. Changes may be made to comply with legal requirements or address business needs.

2. Terms updates

We may update these terms and conditions at any time. The latest version will always be available on our website.

Cancellations and refunds

1. Cancellation policy

  • To cancel or reschedule an appointment, contact us by phone or email at least 48 hours in advance.
  • Cancellations made within 48 hours of the appointment may result in a partial or full deduction of the appointment fee to cover administrative costs.

2. Clinic-initiated cancellations

Vital Leaf may cancel your appointment without liability if:
1. You fail to provide required information within the specified timeframe.
2. You do not pay fees when due.
3. You repeatedly violate our terms and conditions.
4. You breach our Zero Tolerance Policy.

Medication returns

Vital Leaf and our partner pharmacies do not accept returns of medications or offer refunds for dispensed prescriptions.

If you have concerns or wish to provide feedback about our services, please:

  • Complete the feedback form provided after your consultation.

Additional terms

1. Force majeure

Vital Leaf is not liable for delays or failures in service delivery caused by circumstances beyond our control, such as natural disasters or regulatory changes.

We may transfer our rights and obligations under these terms to another organisation. If this happens, we will notify you.

3. Code of conduct

Patients are expected to arrive at appointments appropriately dressed and to adhere to clinic policies.

5. Severability
